LED Lighting Service Request

This will ensure that the Service team can undertake a site visit and carry out the task professionally and efficiently by allowing them to arrive at the site, fully conversant with all the relevant information to undertake the task at hand. Completion of this form constitutes the Schools agreement to liability for costs incurred should the reported issue be due to install or misuse rather than the product. Typical time-frame for a response is 5 working days from return. Please note unless otherwise stated parts will be sent to site ahead of the engineer, if you cannot guarantee successful receipt you must notify upon submitting this form.

*The service partners do hereby reserve the right to, change, delay or cancel any site visit due to circumstances beyond reasonable control. We will not accept any liability for any delay or costs incurred by customer or third-party client in respect of either changing delaying or cancelling a site visit. We reserve the right to replace OR repair at our discretion.

• Emergency Lighting Batteries are excluded from this warranty after 1 year
• PIR and Microwave sensors are excluded from this warranty after 2 years
